Sports Physical
Don't get left on the sidelines this season. Bring your student athlete to the RediMed nearest you, and see how easy and convenient getting a sports physical can be.
RediMed offers convenient locations for student athletes to receive a sports physical. Appointments are not necessary to get a sports physical at any of the four RediMed urgent care clinics in northeastern Indiana. A list of the clinics and hours of operation can be viewed here. Forms must be completed and signed by a parent or guardian before the physical can be performed.

Because minors aren't always aware of family health histories, parents should complete the health history, not the patient. Patients 16 and older can have the physical completed without a parent present as long as the form is completed and signed by the parent. It's also recommended that the parent or guardian provide written permission that accompanies the student athlete to the clinic or that the parent/guardian is available by phone to give consent.
Children under the age of 16 must have a parent/guardian present and the necessary paperwork completed and signed. The form can be obtained above, or one can be printed off in the clinic at the time of arrival. Although no appointment is necessary, you may call ahead to determine a possible wait time based on the number of patients being treated at that moment.
The standard fee for a sports physical is $93. Many insurance companies do not cover sports physicals. However, if the patient pays the full amount of the physical at the time of service, a discount is available.
