Parental Consent Form

Protecting your children when you're not with them means more than getting a sitter. To be absolutely safe, you should provide written authorization for a responsible adult to approve emergency medical treatment should your child need it. Medical center personnel cannot treat a child without parental or guardian consent unless the injuries are life threatening.

Complete and date this form and give it to the adult you have designated each time you are going to be away from your child. Ask that adult to keep the form handy so it is easily accessible in the event of a medical emergency.
